Yes, I really did take a vacation that was not built around a seminar!  Michael and I went to San Antonio, Texas, to visit with family and enjoyed the time very much.

The closest I got to foot-related was a visit to the Siesta Valley Ranch, a lovely Texas Hill Country ranch belonging to the owner of SAS Shoemakers.  You are probably aware of the constant support of Podiatry by SAS, and have seen their booth in all the exhibit halls.  You may not be aware that they have sponsored a non-competitive Volksmarch event at the Siesta Valley Ranch for 13 years, inviting walkers from Texas, the U.S., and beyond to take part in the walk. 

SAS Marketing  representative Bill Armwood invited us to participate in this year's walk, which coincided with our trip to San Antonio.   There were walkers of every age and ability, and lots of rocking chairs for those who weren't up to the trek.   Two routes were laid out, one a three-mile loop, and the other covering six miles.  The trails were up and down, past lakes and across streams, and next to pastures with cattle, buffalo, burros and other critters along the way.  The Texas-style barbecue served afterward was greatly appreciated by all the hungry walkers, and the rain held off until it was about time to leave.

A quick note to congratulate Gaibrielle Hauff, PMAC, and the other officers of the Washington State Podiatric Medical Assistants Association for a very successful meeting on October 5, 2002.  The Free Fall Seminar is a one-day program provided at no charge for WSPMAA members, and with close to 50 in attendance we had a full house!   Topics included "Patient Histories with Attention to Diabetic Work-up";  "Medical Record Laws"; "Professionalism in the Office"; and "CPT Codes and Their Related Procedures", as well as a review of the latest HIPAA information.   Free lunch was also a reality that day, courtesy of Briazz.  Gaibrielle and some of the other participants are pictured at http://www.footzine.com/FZ_8B.htm

I received some emails about a new computer virus that seemed worth noting.  Several individuals, as well as the president of VisionFusion.net, wrote about the new worm virus known as W32.Bugbear, or I-Worm.Tanatos, quoting Symantec Corporation's evaluation of Bugbear as a severe threat. Symantec has posted a downloadable repair on its Web site.  The link is: http://securityresponse.symantec.com/avcenter/venc/data/w32.bugbear@mm.removal.tool.html

Some subject lines for the e-mail are "bad news'', "Membership Confirmation'', "Market Update Report'', and "Your Gift''. Once a machine is infected, a hacker could steal and delete information from it.  The worm replicates itself through a Windows machine's e-mail address book and can attach itself to previously sent e-mail messages.  The worm can also spread through network systems and has keystroke-logging and backdoor capabilities that allow hackers to intercept passwords and gain access to computers over the Internet.   It also attempts to terminate various antivirus and firewall programs, such as the Symantec/Norton antivirus protection.

Bugbear is currently the worst computer security outbreak globally, according to an email to the Associated Press by Mikko Hypponen, manager of anti-virus research at F-Secure Corp. in Helsinki, Finland.  The release states that the worm is expected to last well into next year because many consumers will not realize their computer is infected.  F-Secure also has posted a fix on its Web site, at http://www.f-secure.com/v-descs/tanatos.shtml

For those who do their antivirus maintenance via "patches", Microsoft issued a patch last year, Security Bulletin MS01-027.   And of course the simplest precaution is often the best:
never open unexpected attachments, even from friends, without confirming their legitimacy.

A little caution never hurts....check it out, have your own system scanned for the worm, and
if you find it in your system, share these resources with everyone in your address book.

Understanding the Hows and the Whys

It is difficult to not take it personally when one is corrected and shown that something should be done differently.  Within our practices, a new or even a seasoned employee is often told to do something in a different way and not told why it is important to do it “that way”.  This lack of understanding why “that way” is a better way is what often leads to the feeling of being incompetent.  Rather than taking this as personal criticism, take a step back and see that this is simply a matter of  “all in a day’s work” for trying to make the office move along or for a more efficient work environment.

Much of the feeling of insecurity in these situations comes down to maturity.  As one matures, they learn that in the typical workday your manager or boss is just trying to make the office more efficient or patient-friendly with the decisions they are making.  It all seems to funnel down to what is best for the patient, which should be everyone’s goal in your office.  The managers and doctors in turn should take the time and effort to explain why decisions are made, big and small, within the office.  It is all an issue of respect.
