If you attended the APMA/ASPMA Meeting in Seattle in 1995 you may recall seeing the Seattle landmark "Toe Truck", and perhaps received one of the pink Toe Truck key rings among the other treasures in the assistants' "Goodie Bags".  Since its creation in 1979 the pink-toed tow truck graced the Lincoln Towing location in downtown Seattle.  It certainly got my attention the first time I saw it!  When you visit here again, you'll be able to see the Toe Truck at MOHAI, a local museum, along side other notable area icons.

Hi Gayle,

Here is a suggestion for the office looking for an updated surgical consent:  check with your malpractice carrier, they may have sample forms that contain the appropriate verbiage.

The response regarding covered vs. noncovered foot care was
excellent  -  clearly explained how some areas are gray!

You touch the lives of many and surely mine big time.
 
Regarding the discussion on consents in the last FootZine, I suggest you do a post that everyone should contact their malpractice company for a copy of their suggested (approved?) consent form and process.  I suggest that you contact PICA and ask them for a copy of a consent (and any other materials they want to give you on this subject) they recommend to include as an attachment with your next FootZine.  It would also be helpful to have a sample office note from them on a consent visit.

Hi Gayle.

I've attached 2 consent forms we use at our office.  One is for
nail surgeries and the other is for all other types of surgical procedures.  We also have a separate form in carbon copy that lists all of the possible risks involved with a surgical procedure.  We have our doctor review that form with the patient during the surgical consult appointment.  The patient is then asked to sign the form and they are given a copy to take home.  We review those risks again with the patient prior to having the consent form signed.  I do not have that form in word format but I can fax a copy to you if you would like me to.

re:      Planning for an Office Move

Dear Gayle,

Congratulations on your milestone!!  I think you are great.
I have a question for any office in Washington State (we are in Seattle) who may have moved into a new office (or new address ) recently and could give us tips on how we go about getting our address changed with all the insurance companies, etc. in the  most efficient and speedy way.  Are there specific forms you use to send each one, can you do it by fax?  How do they give confirmation that the change has been made?  Ten years ago Aetna had to certify our office space which took a long time  -  do they or any other insurance companies still do that?  Are there special requirements for Medicare?  I work alone with my husband and all of this will fall on our shoulders, we may be moving soon and will really appreciate any and all information and "tips" from any office who has gone thru this in the last year so I know I have covered all my bases.

Well, I've saved Dessert for the last course.  A Valentine's Day promotion from Godiva Chocolatier is offering a Grand Prize of 52 pairs of Manolo Blahnik shoes plus 52 boxes of Godiva chocolates plus a trip to New York City.  You can be sure that the email with the pictures of the shoe and the chocolates had me taking a closer look.  And yes, you still have a chance to get in the running (but not in those shoes!) -  the contest goes until February 28th.
